What Causes Constant Abdominal Pain After discontinuing Tramadol?

My Wife had a supra-cervical hysterectomy 13 days ago, all was fine until last night. she was prescribed 100 mg. of tramadol every 6 hours, and 600 mg advil every 6 hours , with a 3 hour offset(one or the other every 3 hours) .She stopped taking the tramadol 3 days ago, and none since, but continued with the advil until yesterday afternoon when a sudden severe stomach pain began (no more advil since).During the night last night (4 a.m.) she vomited a few times, and again mid-day today and she said what it produced was dark brown.the most recent vomit event was 11 hours ago(6 a.m.) and the pain had subsided a bit today but now the pain has returned. We are on a remote island hours away from the nearest hospital by ferry, and it is Sunday evening with limited hospital staff on duty even if we can get there

General & Family Physician 's  Response
Hello,

In my opinion, these symptoms, considering also the dark brown color of vomiting content, might be caused by a bleeding gastroduodenal ulcer. Gastroduodenal ulcers in patients get worse and may bleed after surgeries, and the risk increases after taking Advil. Considering the history given by you, I realize that your wife didn't take any anti-acids for the stomach protection. I suggest her to take Omeprazol twice daily (30 minutes before breakfast and before sleeping) for three- four weeks.

She should have frequent and small meals, and also avoid spicy, fried food, fast food, it may help relieve stomach pain. If she doesn't improve she should consult a doctor and have a gastroduodenal endoscopy done, to diagnose the ulcer and further treatment.

Hope I have answered your query.
Let me know if I can assist you further.

Dr. Suela Gjata,
